PDRN and fillers work together to rejuvenate skin by addressing pigmentation, hydration, and volume loss, offering a comprehensive solution for dark circles and dark lips, says Dr Sonia Tekchandani.

Dark circles and dark lips have long been common aesthetic concerns, with various treatment options available to help address these issues. From genetic factors to lifestyle changes, these conditions can affect anyone, regardless of age or gender. In recent years, the science behind treating these concerns has shifted towards regeneration, offering innovative solutions that go beyond traditional methods like PRP and fillers. Dr Sonia Tekchandani, Consultant Dermatologist, Tender Skin International in a talk with Shriyal Sethumadhavan discusses how PDRN (polydeoxyribonucleotide) and fillers work at a cellular level to tackle dark circles and dark lips, delivering significant improvements in both texture and appearance..

The cause!

The primary cause of dark circles often lies in genetics, with many individuals reporting a family history of this condition. Additionally, increased screen time, stress, and poor sleep can exacerbate dark circles. Medical conditions, such as low haemoglobin or allergies, can also contribute. Other factors, like skin irritation from skincare or makeup products, can further aggravate the problem.

Similarly, dark lips may be genetic, but factors like vitamin deficiencies, excessive use of matte lipsticks, and smoking are also common culprits. Smoking, in particular, leads to pigmentation and dryness, which can result in dark, cracked lips.

PDRN glow

PDRN is gaining attention in aesthetic treatments due to its ability to work at a cellular level. It is known for its regenerative potential, stimulating collagen production and improving skin texture. PDRN also neutralises free radicals, reduces melanin production, and boosts hydration in the skin.

For dark circles, the application of PDRN addresses both pigmentation and skin texture. Since dark circles often result from pigmentation or poor skin texture (or both), PDRN can significantly improve the overall appearance by enhancing hydration, reducing pigmentation, and stimulating collagen production. Over time, clients notice a reduction in the appearance of dark circles, with smoother and more hydrated skin around the eyes.

Similarly, PDRN works for dark lips by addressing the skin’s hydration and texture, helping to reduce pigmentation and providing an instant glow to the lips. The procedure is minimally invasive and typically requires a series of treatments. Within a week, clients can see noticeable results, with their lips becoming more hydrated and the pigmentation visibly reduced.

Volumising fillers

While PDRN works well for surface-level pigmentation and texture issues, fillers are the go-to option for more profound concerns, such as deep-set dark circles or hollowing under the eyes. The hollowing effect can cast a shadow, making the dark circles appear more pronounced. Fillers restore volume by filling in the tear trough area and enhancing the skin’s overall appearance.

When combined with PDRN, fillers address both the pigmentation and volume loss, offering a more holistic solution. Clients who have already undergone filler treatments for dark circles often return to address lingering pigmentation issues. In these cases, PDRN can be introduced to further enhance results by reducing pigmentation while maintaining the volume correction provided by the filler.

PDRN vs. fillers

PDRN and fillers offer complementary benefits, depending on the individual’s concerns.

• PDRN: Best for individuals with pigmentation issues or poor skin texture. It works to regenerate collagen, hydrate the skin, and reduce melanin production. It provides gradual improvements over a few months.

• Fillers: Ideal for individuals with deep-set dark circles or hollows. Fillers offer instant results by restoring volume and correcting the shadow effect caused by hollowing.

For clients with both pigmentation and hollowing concerns, combining both treatments can yield the best results. PDRN can be used for pigmentation and hydration, while fillers address volume loss, offering a comprehensive solution.

Pigment repair

One of the standout benefits of PDRN is its ability to target pigmentation issues, especially in delicate areas like the undereye and lips. PDRN’s regenerative properties stimulate collagen production, improve skin texture, and provide an antioxidant boost. In the case of dark circles, this means that the treatment works to lighten the skin and reduce the underlying pigment, resulting in a more refreshed and youthful appearance.

Similarly, for dark lips, PDRN reduces pigmentation by improving hydration and promoting cellular regeneration. Over the course of a few sessions, clients can expect noticeable improvements in the colour and texture of their lips, with results visible within a week.

Unlike traditional treatments, such as PRP and fillers, Polydeoxyribonucleotide addresses the root causes of skin ageing by neutralising free radicals and boosting skin regeneration.

The ideal treatment plan

Both PDRN and fillers are session-based treatments, typically requiring multiple sessions to achieve optimal results. For dark circles, PDRN can be performed initially for pigmentation and hydration, followed by fillers to address deep-set concerns.

For dark lips, PDRN can be used as a stand-alone treatment to improve hydration and reduce pigmentation. For those with more significant pigmentation, PDRN may be combined with other treatments like chemical peels for faster and more effective results.

Assessing contraindications

Before performing PDRN or filler treatments, it is crucial to evaluate for contraindications or skin conditions that could make a patient ineligible. Similar to filler procedures, it is essential to ask about allergies, autoimmune diseases, or active infections in the treatment area. Additionally, pregnancy is a contraindication.

For PDRN treatments, it is recommended to conduct a test patch or intradermal test dose, especially for new clients, as some individuals may exhibit allergic reactions to components like glutathione or PDRN itself.

On the other hand, fillers, being well-tested and commonly used, typically do not require sensitivity tests. However, thorough assessments remain vital to ensure safety and efficacy.

Expected results

Patients can expect progressive but natural-looking results with PDRN and filler treatments. Improvements start with enhanced hydration and a reduction in pigmentation immediately after treatment. Within a week, the skin shows visible changes that continue to improve over two to three weeks.

By the second session, often a month after the first, patients notice significant enhancements. Over three months, collagen buildup and tissue regeneration address minimal tear through hollowing, providing even better results. These treatments work well for individuals hesitant about fillers but seeking improvements in dark circles or minor hollowing.

Myths and concerns

There are several myths surrounding PDRN and filler treatments. Patients often worry about bluish discoloration of the eyes, which can occur with incorrect injection depth, or fear permanent side-effects. These concerns are largely unfounded with proper technique and protocols.

PDRN treatments have proven to have minimal downtime and deliver excellent results. Combining PDRN with mild peels, such as glycolic or lactic acid, enhances surface texture and pigmentation. Non-invasive additions like radiofrequency treatments further improve puffiness and dark circles.

Maintenance and aftercare

Maintenance is crucial for sustaining the results of treatments for dark circles or lips. After completing the initial three monthly sessions, a maintenance session every six months is recommended. Homecare is equally important, involving a skincare regime that includes retinols, antioxidants, and gentle lightening agents.

Aftercare protocols also play a key role in ensuring optimal results. Patients should avoid washing the treated area for 12 hours, use filtered water for the first wash, and refrain from vigorous activities, swimming, or steam exposure for 48-72 hours. Makeup and chemical products should also be avoided for at least 72 hours.

The power of PDRN and fillers

While PDRN works at a cellular level to improve skin texture, hydration, and pigmentation, fillers provide immediate volume restoration for deep-set concerns like hollows under the eyes. The combination of both treatments address both superficial and structural issues. By incorporating regenerative techniques, patients can achieve natural-looking, long-lasting improvements. Regular maintenance and a tailored aftercare routine are essential to maintaining results and ensuring optimal skin health. Individuals seeking to enhance their appearance can expect to see noticeable, gradual improvements, leading to smoother, more youthful skin in sensitive areas.
